  • Deductibles
  • Non-covered services

Self-Pay

Self-pay appointments are available. Please contact our office for current pricing.

Superbill Requests

Patients paying out of pocket may request a superbill, an itemized receipt that may be submitted to their insurance company for possible out-of-network reimbursement.

  • Diagnosis codes (ICD-10)
  • Procedure codes (CPT)
  • Provider information
  • Dates of service

Reimbursement is determined solely by your insurance plan. We recommend contacting your insurance carrier directly to verify your out-of-network benefits and reimbursement eligibility.

Outstanding Balances

Outstanding balances remain the responsibility of the patient and may result in collection activity or interruption of future appointments if left unpaid.

Medication Management

Medication decisions are individualized and based on your symptoms, medical history, treatment goals, and current evidence-based guidelines.

Controlled Medications

Because stimulants, benzodiazepines, and other controlled medications are regulated by federal and Texas law, additional safety requirements may apply.

These may include:

  • Comprehensive psychiatric evaluation
  • Controlled Substance Agreement
  • Routine follow-up appointments
  • Review of the Texas Prescription Monitoring Program (PMP)
  • Urine drug screening when clinically appropriate

Prescription Refills

Please allow 3–5 business days for prescription refill requests.

We are unable to provide:

  • Early refills
  • Replacement prescriptions for lost or stolen medications
  • Refills after business hours, weekends, or holidays

Medication Safety

Patients are expected to:

Superbill Requests

  • Take medications exactly as prescribed
  • Never share prescription medications
  • Inform us of medications prescribed by other healthcare providers
  • Attend required follow-up appointments

Failure to safely use prescribed medications may result in changes to your treatment plan or discontinuation of prescribing.

Laboratory testing may be recommended when clinically appropriate to support safe, personalized psychiatric care.

Testing may include:

  • Advanced biomarker testing
  • Blood work
  • Urine drug screening
  • Medication monitoring

Certain medications including stimulants and benzodiazepines may require laboratory testing before or during treatment.

Failure to complete requested laboratory testing may delay or prevent prescription refills when medically necessary.

Patients are responsible for laboratory costs unless covered by insurance.

Your Privacy

Nest Psychiatry is committed to protecting your personal health information in accordance with HIPAA.

Your information is shared only:

  • With your written authorization
  • For treatment, payment, and healthcare operations
  • When required by law

Telehealth appointments are conducted using secure, HIPAA-compliant technology.

Patients may request copies of their medical records in accordance with applicable federal and Texas laws.

Medical records may also be transferred to another healthcare provider upon receipt of a signed authorization.

Patient Responsibilities

To help us provide exceptional care, patients agree to:

  • Provide accurate medical history and medication information
  • Follow agreed-upon treatment recommendations
  • Use medications only as prescribed
  • Maintain respectful communication with providers and staff

Although uncommon, Nest Psychiatry may discontinue the patient-provider relationship in certain circumstances, including:

  • Repeated missed appointments or excessive late cancellations
  • Failure to follow treatment recommendations
  • Failure to complete required laboratory testing
  • Medication misuse or diversion
  • Abusive, threatening, or inappropriate behavior
  • Outstanding unpaid balances

When appropriate, written notice will be provided (typically 30 days), allowing time to establish care with another provider. Medical records will be transferred upon receipt of a signed authorization.
